Transplant means something different depending on where you are in relation to it. You might be:

- at the beginning, trying to navigate all the new information you have to learn about your upcoming journey.

- on the list, and you are waiting, feeling sicker each day, and  just waiting in the limbo phase. 

- on the other side of surgery, but it does not mean it is any easier because you have traded in one set of problems for another. 

- the person who raised their hand and were a match for someone but now you are left with so much you didn't expect.

- a family member or caretaker on the periphery, holding space for your loved one going through it. 

 

No matter where you stand in relation to transplant, you are carrying something heavy. And you have probably been carrying it quietly for a long time.

I'm Sarah Holmes, a Licensed Clinical Social Worker and Clinical Supervisor offering online therapy for organ transplant recipients, pre-transplant patients, living donors, and caretakers and family members across California.

 

I want to tell you something about why this work exists.

 

I am also an organ transplant recipient. I have walked this

path. I know what it feels like to be seen only through the

lens of one body part, to have your whole identity collapse

into a diagnosis, a procedure, a prognosis. I know the

waiting. I know what it feels like on the other side of the

waiting. And I know how much of yourself gets put on the

back burner when surviving has to take top priority.

 

For over 15 years, I have worked with people navigating

anxiety, addiction, depression, grief, loss, and major life transitions.

But this work, online therapy specifically designed for the transplant

community in California, is where my personal experience and my clinical training meet in a way that feels purposeful. I wanted to take one of the single most challenging experiences of my life and use it to build something real. A space where pre and post transplant recipients, living donors, and the people who love and care for them can finally say the things they have been holding inside.

 

This is not a clinical space that treats you like a chart. It is a space where laughter is welcome, tears are welcome, silence is welcome, and those profound a-ha moments are very much part of the work.

What Therapy Actually Looks Like Here

Online therapy for organ transplant recipients, pre-transplant patients, living donors, and caretakers across California means you can access this space from wherever you are: in your recovery, your waiting, or your caregiving. No commute. No waiting room. Just you, and a therapist who genuinely gets it.

 

Some of what clients work through in this space:

- The grief of the life you had before, and the complicated feelings about the life you have now

- The guilt, relief, fear, and isolation that can come with being a living donor

- The weight of caregiving and what it costs you physically, emotionally, and relationally

- The strange in-between of feeling better but not quite feeling like yourself

- The things you have never said out loud because you did not want to worry anyone

 

Sessions unfold at your pace. I do not assign homework, but I may gently invite you to notice something or try something between our time together. The work goes as deep as you are ready to go.
